Uses of this medication

Gastric Acid Reduction

Nexium - Esomeprazole is a PPI that reduces gastric acid secretion in Gastric hydrogen potassium ATPase through inhibition in gastric parietal cells. By inhibiting the functioning of this enzyme, the Esomeprazole prevents formation of gastric acid.

Helicobacter pylori eradication

When antibiotics clarithromycin and amoxicillin (or metronidazole in penicillin-hypersensitive patients) are combined with Esomeprazole this is referred to as triple therapy for the eradication of Helicobacter pylori. This is a very common treatment for peptic and duodenal ulcers infected by H. pylori.


Dosage Forms

Esomeprazole in pill form is available in strengths of 20 mg and 40 mg. Esomeprazole pills are enteric-coated, due to the acidic conditions of the stomach and rapid degradation of the drug. This is achieving the multiple-unit pellet system of Esomeprazole.


Multiple unit pellet system

Esomeprazole "multiple unit pellet system" (MUPS) are formulated capsules.  Each capsule consists of very small enteric-coated pellets of esomeprazole within an outer shell. When esomprazole reaches the stomach  the contents swell from the water absorption causing the outer shell to burst, releasing the medicine. For most patients, there is no difference with taking the traditional entreric-coated over the multiple unit pellet system.

Patients that require a feeding tube or have dysphagia condition that makes it difficult to swallow may find that the MUPS method of taking Espmprazole more comfortable.


Side effects

Common side effects of Esomprazole may include  headache, diarrhea, nausea, gas, decreased appetite, constipation, dry mouth, and abdominal pain. More severe side effects could include allergic reactions, chest pain, dark urine, fast heartbeat, fever, persistent sore throat, severe stomach pain, unusual bruising or bleeding, unusual tiredness, and yellowing of the eyes or skin.

 

Patients that take PPIs are at greater risk for hip fractures and clostridium difficile associated diarrhea. Frequent administration of the drug in intensive care situations as a protective means from ulcers are 30% more prone to an occurrence of pneumonia.
